The guidelines below are for health professionals looking to refer their client, such as GP, other medical specialist, or practitioner, or care coordinators, or case managers of those funded by a community package (ie NDIS, TAC, LCHS, etc). However, clients can self refer for services, and in general the information is also useful for those who are self referred. They are required to fill in a GNS referral form, and provide relevent medical information as requested on the form.

Referral checklist for medical providers 

 

• ​Briefly explain to the client what is involved in the service being referred for (Clinical Neuropsychological assessment, Initial needs consultation, General psychology session). 

 

• Inform the client that the comprehensive Clinical Neuropsychology assessment is not covered by Medicare, and it takes between 12-25hrs to complete the assessment sessions, and comprehensive report. The cost varies between $3000-$6500, with the average time and cost falling between this amount. It also takes time for the comprehensive report to be completed, and this can take several weeks. If they don't have the capacity to fund the assessment, they can look at sources of community funding that may be available from various institutions to apply for funding.

 

• If the client would like to proceed with a referral, download the GNS referral form, and either assist them with filling it out electronically, or email a copy to their nominated email address & make sure that they are either able to fill it out themselves electronically, or have access to some assistance with filling it out.

 

• If the client is unsure of what their needs are, they can book a 1hr initial needs consultation session, which will provide them with some guidance for ongoing care planning. This session is $250, and a partial medicare rebate is available if the client has a GP mental health plan in place (or charged at the rate set by the organisation if already an existing NDIS or TAC client).

• Email the completed referral form to GNS, along with GP health summary & list of medications, and any other relevant supporting documents/reports. If there are several documents, a copy can be sent to GNS postal address if too much to email.

Referral checklist for Community funded clients: Support/Care Coordinators/Case Managers

• Inform the client that the Clinical Neuropsychology assessment is conducted over a few sessions of between 1-2 hours each, which is approximately 4-8 hours in total. It also takes time for the comprehensive report to be completed, and this can take several weeks. ​

• Gain their consent to refer for services.​

​

• Download the referral form, and ensure the client has assistance with completing the referral.​

​

• Email referral form to GNS along with GP health summary & list of medications, and any other relevant supporting documentation/reports. If there are several documents, a copy can be sent to GNS postal address if too much to email.
